Women's HeartAdvantage 2005 Annual Report in LHF

The success of the mission of the LaPorte Hospital Foundation hinges on two ideas:
community and collaboration. To borrow a phrase.... " it takes a village." The help of many friends and
funding from many sources through the La Porte Hospital Foundation Grant Center enables us to further meet the healthcare needs of the La Porte Community.

In 2005, the 2BigHearts Foundation funded two free heart screenings for 214 women. The mission of the 2Big Hearts Foundation is to heighten awareness of heart disease among women by working with the healthcare community to educate individuals and families about the cause and prevention of heart disease in women. La Porte Hospital Foundation, the Women's Heart Advantage program, Wellness Resource Center, Cardiovascular Services and other hospital departments partner with the 2 Big Hearts Foundation in this valuable work for the benefit of our community. La Porte Hospital Foundation also works in conjunction with the 2Big Hearts Foundation to raise funds for heart disease education and screenings. The 5K Run and Walk, sponsored by the 2Big Hearts Foundation, attracted more than 200 participants from around the country and raised over $14,000.

A 4 Community Health Implementation Grant was obtained in collaboration with the La Porte YMCA to assist funding of a well ness program featuring exercise, nutrition and behavior modification. "Dump Your Plump" is a program which benefits La Porte County families, community members and retail and commercial business members in long-term lifestyle changes for better health.

Healthy Communities of La Porte County accepted proposals by La Porte Hospital Foundation to assist with funding of several tobacco-free education programs. The funds are received from the Indiana State Department of Health and are used to educate inpatients and outpatients on the hazards of smoking and to help businesses in the community create smoke-free work environments. Information regarding the risks of tobacco use are also distributed to the offices of physicians and dentists in the area.

Other grants received by th La Porte Hospital Foundation include one from the Unity foundation to assist with the purchase of scanner/copier by the La Porte Hospital Community Health Center. Other grants from the Matthew Frank Moore Foundation assisted with enhancement of the office facilities of the La Porte Hospital Community Health Center and the La Porte Dental Clinic.

The Programs supported by these grants are important to the health of our community.

Our sincere thanks is extended to those who support our mission.
